Student Information

The Graduate Student Medical & Dental Insurance Plan is provided to eligible graduate student assistants as a benefit. Please contact your appointing department to determine your eligibility. The Plan is designed to help you pay a portion of the medical & dental expenses you may incur. Coverage is available worldwide.

 

2007 - 2008 Plan Information

 

  • Plan Summary - View Plan benefits, including deductible and payment schedule.
  • Dependent Rates - Spouse, domestic partner, and children semester premium rates. Dependent policy does NOT include dental coverage.
  • Dependent Enrollment Form - Dependents must be enrolled on a semester by semester basis. Newly acquired dependents (new spouse/domestic partner, child, adoption) must be enrolled and premium must be paid for them within 31 days after becoming eligible in order to become insured.
  • Domestic Partnerships - View qualifying criteria for domestic partnerships, and download the Affidavit of Domestic Partnership Form.
  • Dental - Dental coverage overview.
  • Eligibility - GSA Insurance Plan eligibility defined.
  • Claim Form - Submit your claims by completing this form and mailing to Maksin.
