The cancellation deadline for Fall or Annual enrollments is September 19, 2008. The cancellation deadline for Spring/Summer is February 6, 2009.

- Domestic Partnerships - View qualifying criteria for domestic partnerships, and download the Affidavit of Domestic Partnership Form.
- Premium Rates - Annual, Fall, and Spring rates for student, spouse, and children.
- Enrollment Form - Newly acquired dependents (new spouse/domestic partner, child, adoption) must be enrolled and premium must be paid for them within 31 days after becoming eligible in order to become insured. The student enrollment deadline for Fall or Annual coverage is September 5, 2008. The student enrollment deadline for Spring/Summer is January 23, 2009.
